BACKGROUND: Bacterial and protozoan Sexually Transmitted Diseases (STD) are common in Mongolia suggesting the potential for rapid spread of HIV. This study evaluated the utility of filter paper blood collection technique for monitoring HIV, syphilis and viral hepatitis prevalence in resource limited settings such as Mongolia.METHODS: Volunteers recruited from several locations in Ulaanbaatar (UB) included commercial sex workers, itinerant traders, homeless persons and hospital inpatients. All subjects completed a questionnaire and provided blood via a finger stick collected as multiple filter paper spots. Filter paper specimens were stored at 4 degrees C and later eluted into saline serologic testing for syphilis, HIV-1, hepatitis B surface antigen and hepatitis C antibodies using commercially available kits.RESULTS: For 618 individuals recruited, the mean age was 29.5 years, with 45% males and 55% females. Prior STD was reported by 27%. The overall prevalence of infection was 1.9% for syphilis, 10.2% for hepatitis C and 21.5% for chronic hepatitis B. Evidence of hepatitis B and C infection were disproportionately common among STD clinic patients and homeless persons, respectively. Disease prevalence by participant subgroups and questionnaire responses will be presented. Three individuals had repetitively positive ELISAs but were negative by Western blot.CONCLUSIONS: In UB Mongolia, rates of chronic hepatitis B and hepatitis C were high while syphilis rates were lower than expected. No cases of HIV were identified. The filter paper approach for blood collection is useful for serological studies in settings where blood collection and/or specimen transport is difficult. Specimens were easily collected, stored, and transported prior to testing.
